This feedback form is designed for teacher-powered teams who are familiar with teacher-powered practices and teacher-powered autonomies. The goal is to provide feedback to teams on their collective skills and implementation of teacher-powered practices and autonomies. Please answer the survey questions for the current school year. Each year teams will have a chance to take the survey in the spring to see their growth and provide data for reflection.

Each team will receive their survey results to review their strengths and identify areas for growth as well as individualized resource suggestions to use with your team based on your specific school’s data.

Anyone participating in school level decision-making is welcome to take the survey including administrators, teachers, para professionals, etc.
